Appointments

If you are having health problems, we encourage you to call early in the morning so we can give your problem attention the same day.  You are encouraged to describe the nature of the problem to the receptionist so that she may schedule a convenient appointment.

If the problem you describe involved more than one member of your family you need to tell us, so we can allow extra time for the extra person.

We try to stay on time, but occasionally we have emergencies which tend to put the schedule behind.

Cancellation Policy

Patients must call to cancel their appointments 24 hours in advance or will receive a $25 charge.

When You Arrive

When you arrive at our office for a first visit, the receptionist will ask you to complete a "New Patient Information Sheet." To save time and fill out the form before you come in, click here.   These questions are a necessary part of your examination.  Please be assured that all information is confidential and none of your records will be disclosed without your consent.  We also request that you keep our office aware of any name or address corrections.

Waiting Room

We have provided adequate seating for our patients.  Please be considerate of post-operative patients and the elderly by allowing them to be seated.  For comfort of everyone, we ask that only one person accompany the patient who is to see the physician.
